Documents required for the form:

  1. Passport Copies: Scanned images of your signed passports.
  2. Proof of Address for each applicant (PDF): This can be the first page of a bank statement, utility bill, or driver’s license. 
    The document should include your name, as it appears on your passport, and your physical address.
  3. Proof of Employment for each applicant 
  4. Foreign Tax Identification Number of each applicant
